How to register at the practice?

Please check that you are living within the practice area using the map

We welcome new patients to the practice.

You do not need to come to the practice to register as a patient.
Please complete the two registration forms below and return them to us with your photo ID plus two documents with your current address on - photos of the completed forms/documents are acceptable.

If you are unable to download or print the documents please phone us on 0117 944 9750 and we will send the forms to you in the post.

We accept the following documents to complete your registration

Identification / Documentation - One piece of photographic ID which can be:

  • Passport
  • Photo driving Licence
  • Firearms Licence (UK)
  • EU National Photo Identity card
  • HM forces ID Card

Or

  • Your original Birth Certificate

Plus two documents that evidence your current address (dated within the last 3 months):

  • Statement of Benefit
  • Tenancy agreement/mortgage statement
  • Utility bill including TV Licence and Virgin/Sky Accounts
  • Bank Statement/Post office account
  • Phone bill or mobile phone contract
  • Council tax bill
  • Wage slip with address on

If you DO NOT have photographic ID or your birth certificate, we can accept THREE documents with your current address identified, from different organisations dated in the last three months.

If you think you may have difficulty providing this documentation, please ask to speak to the practice manager.

E-mail Communication

We provide patients with the option of communicating with the practice by e-mail for NON-URGENT concerns. If you would like to give consent for us to communicate with you by e-mail please download and fill out our consent form >

Consent for other family members to access your records

If you would like other family members to act on your behalf and discuss your medical record please print and complete this form Patient consent to release information form > - We will accept a photo of the completed form which can be emailed to merrywoodpractice@nhs.net

On line access

If you are over 16 you will automatically be given an account with PATIENT ACCESS which will enable you to book appointments and order prescriptions online. Please click below for more information on this.
