How to register at the practice?

In order for patients to register with the practice during the current Coronavirus pandemic, patients can e mail or post their completed registration documents, photo ID and address verification to us, without having to come to the practice

We welcome new patients to the practice.

What you'll need to bring with you:

Identification / Documentation - One piece of photographic ID which can be:

  • Passport
  • Photo driving licence
  • Firearms Licence (UK)
  • EU National Photo Idenditiy card
  • HM forces ID Card


  • Your original Birth Certificate

And Two pieces of Evidence of current address (dated within the last 3 months) can be:

  • Statement of Benefit
  • Tenancy agreement/mortgage statement
  • Utility bill including TV licence and Virgin/Sky Accounts
  • Bank Statement/Post office account
  • Phone bill (not a mobile)
  • Council tax bill
  • Wage slip with address on

If you DO NOT have and photographic ID or your birth certificate we can accept THREE documents with your current address these must be from different organisations in the last three months. If you think you may have difficulty providing this documentation, please ask to speak to the practice manager.

E-mail Communication
We provide patients with the option of communicating with the practice by e-mail for NON-URGENT concerns. If you would like to give consent for us to communicate with you by e-mail please download and fill out our consent form >

What to do next?
Once you have the necessary ID & documentation and have filled out the forms, please hand all of this in to reception, we will then get you registered with us over the next few days.
