Identification / Documentation - One piece of photographic ID which can be:
Photo driving licence
Firearms Licence (UK)
EU National Photo Idenditiy card
HM forces ID Card
Your original Birth Certificate
And Two pieces of Evidence of current address (dated within the last 3 months) can be:
Statement of Benefit
Tenancy agreement/mortgage statement
Utility bill including TV licence and Virgin/Sky Accounts
Bank Statement/Post office account
Phone bill (not a mobile)
Council tax bill
Wage slip with address on
If you DO NOT have and photographic ID or your birth certificate we can accept THREE documents with your current address these must be from different organisations in the last three months. If you think you may have difficulty providing this documentation, please ask to speak to the practice manager.
Registration forms You will also need to complete two registration forms which you can either collect from the reception desk or download here:
E-mail Communication We provide patients with the option of communicating with the practice by e-mail for NON-URGENT concerns. If you would like to give consent for us to communicate with you by e-mail please download and fill out our consent form >
What to do next? Once you have the necessary ID & documentation and have filled out the forms, please hand all of this in to reception, we will then get you registered with us over the next few days.
