Mar 20 The Merrywood Practice will now accept New patient registrations via email
We are in the process of updating this part of the website. Both the completed forms and copies of photo ID and any proof of the address (please see below details of acceptable forms of ID and proof of address please send to email@example.com.
Please complete all of the below if you are wanting to become a permanent patient at the practice
Identification / Documentation - One piece of photographic ID which can be:
Photo driving licence
Firearms Licence (UK)
EU National Photo Idenditiy card
HM forces ID Card
Your original Birth Certificate
And Two pieces of Evidence of current address (dated within the last 3 months) can be:
Statement of Benefit
Tenancy agreement/mortgage statement
Utility bill including TV licence and Virgin/Sky Accounts
Bank Statement/Post office account
Phone bill (not a mobile)
Council tax bill
Wage slip with address on
If you DO NOT have and photographic ID or your birth certificate we can accept THREE documents with your current address these must be from different organisations in the last three months. If you think you may have difficulty providing this documentation, please add this fact to your email to the practice.
