About patient online access Patient access allows patients to book appointments and order repeat prescriptions online
You have the option to view your Summary Care Record - this will include a summary of information relating to medicines you take, allergies you suffer from and any bad reactions to medicines you have had.
You can request access to additional coded data - this allows access to more detailed parts of your medical records. If you would like this service please ask at reception for an application form >
NEW July 19 Proxy Online access is now available for under 16's and those who have a carer if you would like proxy access - please complete this Proxy access request form > and return it to the practice.
New Patients We are now automatically enrolling new patients onto PATIENT ACCESS and they will receive their online login details in the post. If you are a new patient and do not wish to have this service just let us know.
Want Patient Online Access? If you are already a patient and do not yet have PATIENT ACCESS you can register by bringing in your ID, please bring along some photo ID. Mar 20 - patients can now email photo ID to the practice for their account to be set up email@example.com
Please note that you will need to provide photographic ID and must be aged 16 or over.
